IMPORTANT UPDATE: MI Marijuana Program Changes Paperwork

For all patients, physicians, caregivers and dispensaries across the State of Michigan, be aware that new forms are available on LARA’s website, view here >>

All old copies should be thrown out, or better yet, recycled. The new forms reflect legislative changes to the Act that went in to effect this year and involves a few more items to be completed.

Major changes impact caregivers and physicians, while patients now need a State of Michigan picture ID or Michigan voters registration with their application. No out-of-State licenses accepted.

On a brighter side, cards will know be valid for 2 years and fees are still the same, for the time being.
